With the holidays approaching in a few short months, you may be wondering when’s the best time for scheduling aesthetic treatments. You should always try to err on the side of caution and book aesthetic treatments with enough wiggle room for downtime, possible touchups, and minimum result time in mind. This is especially true for events with fixed dates, such as the holiday season, a wedding, or a reunion.

Injectable Treatments (Neurotoxins, Fillers, Skin Booster Injections)

 

Neurotoxin injectable treatments like Botox, Dysport, Jeuveau, or Xeomin take up to two weeks to be fully effective for reducing facial wrinkles. In large muscle areas like the traps (shoulders), masseters (jawline), or calves, it can take even longer to be evident, like 3-4 weeks.

Dermal filler treatments take several days to weeks to settle and for post-treatment swelling to go away. Treatment with dermal fillers should be thought of as a marathon, not a sprint, so desired results may not be achievable in one treatment. This is because treated areas generally need time to acclimate filler volume; larger amounts of filler may need to be spread across multiple appointments. If touch ups or subsequent treatments are necessary, they generally need to be scheduled a minimum of 2 weeks after an initial appointment.

Skin booster injections may come with minor scabbing if the entire face is treated.

All injectable treatments come with risk of bruising or swelling for several days to weeks after treatment. Bruising is especially likely with long term use of blood thinners, baby aspirin, or fish oil supplementation.

Because of all these factors, we recommend getting skin booster injections a minimum of 3 weeks – 1 month in advance, neurotoxin treatments a minimum of 1-2 months in advance, and dermal filler treatments a minimum of 2-3 months in advance.

 

Energy-Based Treatments (Thermage, Ultherapy, HIFU, Radiofrequency Skin Tightening, etc.)

 

While energy-based skin treatments are generally minimally-invasive, they do require time in order to build collagen and lift and tighten skin. Skin will continue to improve several months after treatment, which is why it is critical to schedule these treatments in advance. Some people may even choose to do repeat treatments after a few months on local areas that need further improvement. While these treatments generally have minimal downtime, there may be some swelling or welting post-Ultherapy treatment for several days. It is recommended to schedule these types of treatments 2-5 months in advance.

 

Microneedling (Morpheus8, Genius)

 

Microneedling treatments offer the best rejuvenation results after a few treatments. There is downtime of a few days for swelling and scabbing to improve post-treatment, especially with RF microneedling treatments like Genius or Morpheus8. Skin will continue to improve for 90 days or more post-treatment, which again means that time is necessary for the best results to be evident. Try to schedule your microneedling treatments so your last session falls a minimum of 2-3 months in advance.

 

Laser Treatments

 

Because laser treatments vary so widely in terms of laser type, intensity, and purpose, different treatments will necessitate different scheduling requirements. Understand that the more ablative a laser treatment, the longer the downtime and recovery process. Many laser treatments also require several treatments, such as in the case of laser hair removal, treatment of pigmentation, and especially the treatment of texture, pores, or acne scarring. Most laser treatments come with at least a few days of downtime. This can include anything from redness, irritation, peeling/flaking, scabbing, swelling, discomfort, or even bleeding/oozing in the case of more intense laser treatments.

When in doubt, schedule earlier! For a minimally invasive treatment like Pico laser toning, try to treat your latest appointment at the very least 2 weeks in advance. For a laser treatment like LaseMD Ultra, Fraxel, or a fractionated Pico laser treatment, try to schedule your last appointment at least 2 months in advance. For more intense laser treatments, try to schedule 3-5 months in advance. For very ablative CO2 laser or any very strong laser treatment that can cause lingering hyperpigmentation, it’s best to treat 6 months – 1 year in advance.

Fall is almost here! Here’s some autumn skincare tips on how to change up your skincare for the cooler months approaching.

  1. Switch back to a more hydrating cleanser. Foaming cleansers are great for the heat of summer, but if they start feeling a bit too stripping, consider changing to a more gentle cleanser or an oil-based cleanser. Cleansing milks or balms are great options to remove all makeup and sunscreen without leaving skin feeling too dry.
  2. Switch from a gel or light moisturizer to a thicker moisturizer. As the weather grows colder, lightweight formulas may not be moisturizing enough. Thicker or heavier formulations are great to keep skin plump and hydrated.
  3. Incorporate an oil into your routine. Adding an oil or a richer moisturizer on top of more water-based products can help prevent transepidermal water loss (TEWL) to keep skin moisturized and lock in hydration.
  4. Keep applying sunscreen! It can be tempting to use less sunscreen when sunshine isn’t as apparent, but UV rays are still present in cloudy weather. Keeping up with SPF all year round is important for anti-aging due to UV exposure, prevention of skin cancer, and to keep pigmentation at bay. Fall and winter are the best times to do laser treatments, so it’s important to keep using SPF as a way to ensure best results. Try a physical + chemical sunscreen or all chemical sunscreen if you’d like a more hydrating or thin formula.
  5. If your skin or nose is starting to feel a little dehydrated or chapped, autumn is a great time to break out the humidifier. Regular scrubbing + use of distilled water in a humidifier keeps your skin and mucous membranes comfortable and happy.

Dysport

 

Galderma Aesthetics’ answer to Botox is Dysport. Dysport is also an injectable neurotoxin treatment for the temporary relaxation of muscles that cause wrinkles. Effects lasts 3-4 months; however, dosing is different from dosing for Botox. This is due to a different saline reconstitution ratio. Understanding this difference makes for informed decision-making when it comes to deciding the number of units necessary for treatment. 10 Botoxunits is equivalent to about 20-25 Dysport units. This does not mean that Botox is “stronger” than Dysport, merely that the dosing is different.

Sculptra

 

Sculptra is a dermal filler treatment for the treatment of large areas of volume deficiency in the face or body. Examples of great areas for treatment are the temples, lateral cheeks, and even the hips. Sculptra is made of PLLA, or Poly-L Lactic Acid. It is NOT a hyaluronic acid dermal filler like most other fillers on the market. This substance stimulates fibroblast and collagen production in areas of treatment. Skin booster injections. What are they? In English, there doesn’t seem to be one catch-all term for all-over skin injectable treatments. You may have heard of them by the nebulous term “skin booster injections,” by other terms like “AquaGold,” “Baby-Botox facials,” “hydro lift,” “mesoinjection,” or “microinjection” or even by their brand names, e.g. Rejuran Healer, Volite, or Filorga. Because there isn’t a standard name for this type of treatment like there is in Chinese, “水光針,” it can often be quite confusing to even understand what these treatments entail.

 

What is it?

 

In short, skin booster injections are micro-injections of solution cocktail all over the skin. The goal is to hydrate the skin, improve fine lines/texture, shrink pores, and improve the appearance of skin. Infusing various rejuvenating components into the skin improves the skin overall, hence the term “skin booster.” What is injected and how its injected is how skin booster injections differ from each other. The treatment name is broad because it encompasses a wide range of inclusion possibilities.

In general, the solution base is dilute hyaluronic acid (HA). Yes, the same base ingredient as dermal fillers! HA is a sugar molecule that binds to water. It is found naturally in the skin and used in many topical skincare products to hydrate the skin. Dermal fillers and skin booster injections use different types of hyaluronic acid. Skin booster injections generally utilize non-crosslinked HA, while dermal fillers generally use crosslinked HA (although this does not hold true for every type of filler or skin booster).

Crosslinking refers to the hydrogen bonds between HA molecules; the more there are, the more the HA molecules hold their shape and structure. Crosslinking also allows HA to resist degradation/breakdown over time. This is what causes HA dermal fillers to be a thick, gel-like consistency. The more crosslinking, the thicker. This is a great property for dermal fillers, where you want injections of HA to maintain their integrity and stay in place to volumize or change the contours of the face. Skin booster solutions tend to use non-crosslinked, more dilute HA, which allows for delivery all over the skin. No crosslinking allows for better spread of the HA throughout the skin. In comparison to dermal fillers, skin booster injections tend to be a watery or syrupy consistency.

What else besides hyaluronic acid can be included?

 

Besides HA, many inclusions are possible, making skin booster injections highly customizable. Peptides, vitamins, amino acids, growth factors, and PDRN can be added to the solution cocktail. Different brands of ready made solution cocktails are on the market, such as Rejuran Healer or Filorga. Each brand will feature its own selling point, such as PDRN, amino acids, or special combination of vitamins. Personally customized cocktails are also possible, depending on skin concern.

It is also possible to have solutions that contain no HA at all. Depending on the desired results, different combinations can be custom created. For example, “BabyBotox” facials or “microBotox” is a possibility. “BabyBotox” or “microBotox” refers to hyperdilute Botox that is injected very superficially into the skin in order to temporarily shrink pores and give the skin a beautiful, shiny, & smooth appearance. In comparison to traditional Botox injections, many less units are needed. However, many more injection points are required if the goal is to shrink pores; hence its qualification as a form of “skin boosting injection.” “Baby-Botox” refers to the small amounts of Botox needed per injection area.

While normally Botox is injected deeper to target the larger muscles of the face that contract and cause wrinkles, Baby-Botox targets the more superficial and tiny muscles of the pores of the face. While it is true that pores do not “open” or “close,” they do have tiny muscles called arrector pili. Pores are openings in the skin for hair follicles and sebaceous secretions. Arrector pili muscles are responsible for the hairs (whether the hair is visible or not) in each pore standing up when you are cold, which is also known as goosebumps. Injecting Botox to target these muscles relaxes these muscles and temporarily reduces oil secretion from the pore, making pores appear tighter and smaller. This method of injecting Botox also can smooth fine lines over the entire treatment area.

A Baby-Botox facial is different from traditional Botox injections; they do not each have the same effect. The first targets more superficial muscles and has benefits for the appearance of the superficial skin, while the latter targets deeper, larger facial muscles responsible for the areas that form wrinkles.

PRP can also be considered a type of skin booster injection. PRP stands for platelet-rich plasma, which is derived from your own blood. In order to obtain PRP, blood is drawn and centrifuged in order to separate out platelet-rich plasma. Plasma is the liquid component of blood and is rich in platelets and growth factors. These components stimulate body regeneration and wound healing. Injecting PRP back into the skin can boost skin texture, clarity, and appearance by plumping and regenerating skin. PRP can also help improve hair health.

 

How are they administered?

 

HOW skin booster injections are delivered are also where treatments can differ. Skin booster injections can be administered with a standard syringe and needle all over the skin. This takes much more time, but is fantastic for area-specific treatments and navigating some of the trickier contours of the face.

Secondly, a micro-injection gun with several needles can be used, which allows for even depth penetration and regimented delivery amount per “shot.” Much like microneedling, a disposable needle grid tip is used to penetrate the skin in a stamping motion. The microneedling-like tip provides the added benefit of collagen neogenesis due to the act of stamping the needles into the skin. This kickstarts skin regeneration. Unlike microneedling, the needles are hollow, which allows the solution to pass through the needles. The needle tip is attached to a syringe and the gun uses negative pressure to deliver solution cocktail into the skin.

Another delivery method is something like AquaGold, which is a brand of micro-injection device that has a liquid reservoir attached to a needle delivery system. Stamping the needles on the skin allows the solution in the liquid reservoir to pass into the skin.

 

What are the benefits of skin booster injections? What do different add-ons do?

 

The benefit is in the name! Skin booster injections help to enhance and improve skin overall. Injection all over the skin has a benefit of skin rejuvenation through the incitement of collagen growth due to the wound healing process. This is the principle behind microneedling skin treatments. Using needle delivery of skin-benefiting components allows bypassing of the skin barrier and targeted delivery of these compounds. Depending on molecule size, some elements cannot topically pass through skin by themselves. Systemic delivery (such as by mouth or by IV) may not bring rejuvenation to desired areas of improvement. Infusing skin with beneficial additives, such as antioxidants, HA, or skin-regenerating compounds helps to improve skin appearance, tone, texture, elasticity, bounce, and clarity.

Hyaluronic Acid: Hydrates and plumps skin. Helps diminish fine lines due to dehydration.

Glutathione: Skin antioxidant, brightening and helps with skin radiance.

Vitamin C: antioxidant, skin brightening, helps fade post-inflammatory hyperpigmentation.

Polydeoxyribonucleotide or PDRN: PDRN is DNA fragments, usually derived from salmon sperm cells. PDRN has anti-inflammatory, anti-melanogenetic, bone regenerative, tissue damage preventive, anti-ulcerative, wound healing, and scar preventive effects.

PRP: helps to regenerate and revitalize skin, can help improve hair health and density.

Baby-Botox facial, Skin Botox, Micro-Botox, Meso-Botox: Shrinks pores, reduces oil production, and smooths tiny fine lines.

 

How does a treatment go?

 

At DSC, we first determine what goals you have for treatment in order to determine the best cocktail for your skin. Topical numbing cream will be applied to the skin. After enough time has elapsed, the numbing cream is removed and treatment will begin. Solution will be injected into the skin, whether it be with a needle, injection gun, or micro-injection device. Once all solution has been delivered, a cooling mask and/or ice packs will be applied to the face to calm and soothe skin.

 

Is there any downtime?

 

Because these treatments involve microneedling-style stamping or hand injection all over the face, there may be a few days of pinprick scabbing, bruising, or tenderness.

 

Is there any aftercare?

 

It is best to avoid heavy exercise, steam rooms/saunas for a few days after treatment. Skip use of active topicals like AHAs or retinoids a few days before and after treatment. Avoid hot water, alcohol, and makeup for about 24 hours post-treatment. Let skin heal naturally, do not pick, scratch or otherwise disturb skin in treatment area. Wait a minimum of two weeks before and after treatment for facials, massage, waxing, or other treatments in the same area.

 

 

How often do you need treatment?

 

This is highly variable and depends on the type of skin booster you pick. Botox and other brands of toxins are limited to once every 3-4 months. For other combos, it is truly up to your personal preference. While there may be a few days of minor downtime post-treatment, there is no upper limit to skin infusion of vitamins, HA, PDRN, or other add-ons. You can choose to treat once a month, once a quarter, or before special occasions.

What is Coolsculpting?

Coolsculpting is an FDA-approved treatment for non-surgical fat reduction. Coolsculpting utilizes cryolipolysis, or fat freezing, to eliminate stubborn fat cells without damage to other tissue or surrounding areas. Freezing fat cells eliminates them permanently, which can lead up to 20 – 25% reduction of stubborn fat in a treated area.

CoolSculpting is FDA-cleared to eliminate stubborn fat in 9 areas: under the jawline, under the chin, the upper arms, back fat, bra fat, the flank area (love handles), abdomen, thighs, and under the buttocks (banana roll).

The Coolsculpting device comes with several handpieces in order to treat the contours of different areas. Each single treatment lasts between 35 – 75 minutes depending on the handpiece and is known as a “cycle.” Paired areas, such as the flanks, arms, or thighs, generally require at least two cycles, one per side. The anterior abdomen may require more cycles for even fat reduction over the entire area. Of course, treatments will differ depending on individual anatomy and contours.

Our Coolsculpting experts will do an individualized body assessment before treatment begins. Our treatment providers are looking to map and mark handpiece placement for optimal results. This assessment allows an estimate of how many treatments may need to be done or which handpiece is best for a specific area. Any overlapping areas should optimally be treated on the same day in order to eliminate the possibility of uneven fat reduction in a single treatment area, especially in the anterior abdomen. Retreatment of any area should not be any sooner than 4-8 weeks.

 

What is a Coolsculpting treatment like?

Before treatment, a pre-treatment wipe is used to cleanse the area and to protect superficial skin from damage. A gel film with a sticky consistency is applied over the treatment area. The Coolsculpting handpiece will be applied on the treatment area, which will vacuum tissue into the handpiece in order to be treated. If there is no pinching from improper placement and the handpiece has been secured with straps to prevent pop-offs, treatment begins. The first 5 minutes of the Coolsculpting treatment may come with a tingling and minimal discomfort as temperature is reached. After the initial first few minutes, treated tissue becomes numb and any possible discomfort recedes. During this time, you can be on your phone or enjoy yourself as treatment time elapses.

After the completion of the treatment, the handpiece and gel film are removed and the treated area will be firmly massaged for at least two minutes to stimulate lymphatic drainage, ensure return to body temperature, and to break up fat cells even further. Subsequent cycles can be done on different treatment areas. Results can be typically be seen in 1- 3 months, as frozen fat cells are processed and eliminated naturally by the body. Full results may take up to 6 months. If further treatments are desired, the same area can be treated after at least 4 – 8 weeks.

How many treatments are necessary per area?

The number of treatment cycles required to completely treat an area depends on the natural contours of the body. Handpieces are centered over “peaks” of fat pockets. For smaller areas, like the chin, arms, or thighs, this is generally a relatively straightforward placement. For larger planes like the anterior abdomen or flanks/love handles, placement of handpieces can vary widely depending on how “peaks” are situated. Broader areas may be suitable for one cycle with a larger handpiece. If there are multiple peaks in a broader area, this will necessitate several cycles with a smaller handpiece.

This is why assessment is important to estimate the number of cycles needed to treat an area to completion. Repeated treatment sessions in a single area may also be necessary for best results depending on the amount of subcutaneous fat present initially. Because each cycle can lead to up to 20-25% reduction in fat in an area, the more fat there is initially, the more repeat sessions are required. Retreating an area generally leads to more satisfactory results.

Is there any downtime?

Coolsculpting minimally invasive so there is no downtime. There may be slight soreness/numbness in a treatment area for days to weeks after treatment. Some slight swelling may be present for several days after treatment.

Who is a good candidate for Coolsculpting?

Coolsculpting is a body contouring treatment, not a weight-loss treatment. This is because Coolsculpting works non-invasively and only targets stubborn pockets of subcutaneous fat. This is fat that is more superficial to the skin and not deep inside the body. It does not treat visceral fat, which is fat that surrounds organs in the torso. The best candidates for Coolsculpting are those with good diet and who regularly exercise but would like to treat stubborn areas of fat.

Who is NOT a good candidate for Coolsculpting?

Persons who are pregnant or those with cryoglobulinemia, cold agglutinin disease, or paroxysmal cold hemoglobulinuria should not receive Coolsculpting treatments.

 

Can you pair Coolsculpting with other body contouring or body sculpting treatments?

Absolutely. Results can be augmented when you pair Coolsculpting with other treatments such as LPG endermologie, Velashape, or even body sculpting treatments like Accufit. If  an area does not have enough fat to be treated successfully with Coolsculpting, another body contouring treatment may be appropriate. If fat loss and muscle improvement is desired, pairing Coolsculpting and Accufit can synergistically augment results.
